What Is Dialysis Companion Support?

Medicare and private insurers are increasingly supporting home dialysis — but going home doesn't mean going it alone. Many patients and families feel anxious about managing dialysis sessions without clinical backup nearby.

CareNexus provides trained dialysis companion caregivers who are present during every session. We are not operating the machine — we are the extra set of eyes, the vital sign monitor, the kidney-friendly meal preparer, and the first call to your nephrologist if anything looks off.

Pillar 1

Infection Control & Access Site Monitoring

Dialysis access sites — fistulas, grafts, and catheters — require vigilant observation. Our caregivers are trained to identify early warning signs of infection and escalate immediately.

  • Visual inspection of fistula, graft, or catheter site
  • Monitoring for redness, swelling, warmth, or discharge
  • Immediate escalation to nephrologist or care team
  • CDC aligned infection prevention protocols

Pillar 2

Vital Sign Tracking & Documentation

Blood pressure fluctuations and weight changes are critical indicators during dialysis. Our caregivers track and document every session so your medical team has a complete picture.

  • Pre and post session blood pressure readings
  • Daily dry weight monitoring and trending
  • Heart rate and temperature observation
  • Session notes shared with care team

Pillar 3

Kidney Friendly Nutritional Support

Diet is one of the most critical and most difficult aspects of managing kidney disease. Our caregivers are trained in renal dietary guidelines and can prepare meals that protect kidney function.

  • Low sodium, low potassium, low phosphorus meal prep
  • Fluid intake monitoring and tracking
  • Pre and post dialysis snack preparation
  • Coordination with patient's renal dietitian

Pillar 4

Post Treatment Recovery and Fall Prevention

The hour after dialysis is the highest risk window for falls, dizziness, and dehydration. Our caregivers stay present through the full recovery period — not just the session itself.

  • Assisted transfer from treatment chair to bed or sofa
  • Post session blood pressure and dizziness monitoring
  • Hydration support and comfort care
  • Observation for 60 minutes after treatment

Who This Service Is For

Home Hemodialysis Patients

Patients using NxStage, Fresenius, or other home HD systems who want a trained companion present during every session.

Peritoneal Dialysis (PD) Patients

Patients performing daily PD exchanges at home who need assistance with monitoring, documentation, and safety oversight.

Families of Dialysis Patients

Family members who cannot always be present during sessions and need a trusted, trained professional to fill that role.

Patients Transitioning from Clinic to Home

Newly discharged dialysis patients who are nervous about the transition and need extra support and confidence building.

Important: What Our Caregivers Do Not Do

CareNexus Dialysis Companion Support caregivers are trained personal care aides — they are not licensed dialysis technicians or clinical staff. They do not connect or disconnect dialysis machines, administer medications, or perform any clinical procedures. All dialysis treatment is performed by the patient through self care, or by a licensed clinical professional. Our role is safety monitoring, vital sign observation, nutritional support, and recovery assistance after treatment. Our services are supervised by our Director of Nursing.
