Who We Are
CareNexus Home Care Services LLC ("CareNexus," "we," "us," or "our") is a licensed Residential Service Agency (RSA) providing in-home care services in Maryland. We are committed to protecting the privacy of your health information and complying with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its implementing regulations.
This Notice applies to all health information we create, receive, maintain, or transmit in connection with the care and services we provide.
Our Duties
We are required by law to:
- Maintain the privacy of your protected health information (PHI)
- Provide you with this Notice of our legal duties and privacy practices
- Notify you following a breach of your unsecured PHI
- Abide by the terms of the Notice currently in effect
How We May Use and Disclose Your Health Information
Treatment
We may use and share your health information to provide, coordinate, or manage your care and related services. For example, we may share information with physicians, nurses, or other caregivers involved in your care.
Payment
We may use and share your health information to bill and receive payment for the services we provide. For example, we may share information with your insurance company, Medicaid, or other payers to obtain reimbursement for your care.
Healthcare Operations
We may use and share your health information for our internal business operations, including quality assessment, staff training, audits, and compliance activities. This helps us improve the quality of care we deliver.
Referrals and Care Coordination
When a hospital discharge planner, social worker, physician, or other healthcare professional submits a referral on your behalf through our website or by other means, we receive and use that information solely to coordinate and arrange your care. This is a permitted disclosure under HIPAA's Treatment provisions.
As Required by Law
We will disclose your health information when required to do so by federal, state, or local law, including disclosures to public health authorities, law enforcement, or in response to a court order or subpoena.
Emergency Situations
We may share your information in an emergency to protect your health or safety or the health or safety of others.
Uses That Require Your Written Authorization
Other uses and disclosures of your health information not described in this Notice will be made only with your written authorization. You may revoke that authorization at any time in writing, except to the extent we have already relied on it. Uses requiring your authorization include:
- Most uses and disclosures of psychotherapy notes
- Uses and disclosures for marketing purposes
- Sale of your health information
- Any other use or disclosure not described in this Notice
Your Rights Regarding Your Health Information
Right to Access
You have the right to inspect and obtain a copy of your health information that we maintain. We may charge a reasonable fee for copies. To request access, contact us in writing at the address below.
Right to Request Amendment
If you believe your health information is incorrect or incomplete, you may request that we amend it. We may deny your request under certain circumstances and will explain our reason in writing.
Right to an Accounting of Disclosures
You have the right to request a list of certain disclosures we have made of your health information in the past six years, other than disclosures for treatment, payment, or operations.
Right to Request Restrictions
You may request that we restrict certain uses or disclosures of your health information. We are not required to agree to your request, but if we do, we will honor that restriction unless it is needed for emergency treatment.
Right to Confidential Communications
You may request that we communicate with you about your health information by alternative means or at an alternative location (for example, by calling you at a different phone number).
Right to a Paper Copy of This Notice
You have the right to receive a paper copy of this Notice upon request, even if you have agreed to receive it electronically.
How We Protect Your Information
CareNexus uses administrative, physical, and technical safeguards to protect your health information from unauthorized access, use, or disclosure. Our website uses SSL/TLS encryption for all data transmission. Information submitted through our online forms is stored in a secure, access-controlled database. Only authorized CareNexus staff with a legitimate need to know have access to your information.
How to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with CareNexus or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

Changes to This Notice
We reserve the right to change this Notice at any time. We reserve the right to make the revised Notice effective for health information we already have about you as well as any information we receive in the future. We will post the current Notice on our website and make copies available upon request. The effective date at the top of this Notice indicates when it was last revised.
